Information that you provide us about your health history and your current condition in your intake form and in person at your treatments is confidentially recorded and accessed by your practitioner/s.
If you see multiple practitioners at our clinic, your intake form and any clinical notes from previous appointments will be available to the practitioner responsible for providing your treatment on the day. This ensures our ability to provide you with continuous care that doesn't require you to repeatedly explain your health history or injury.
Any additional documents you provide us, such as imaging reports, specialist referrals, treatment notes from previous clinics, etc, will be stored on your patient file and can be accessed by practitioners providing you treatment.
Your practitioner may discuss specifics of your musculoskeletal condition with senior staff members if they require advice on how to proceed to get you the best outcome for your current condition.
As part of our important treatment quality control procedures, your treatment notes may be reviewed by the Clinic Owner to ensure your practitioner is providing a high standard of care to you. You may opt out of this at any time, either by notifying your practitioner at your appointment or by emailing [email protected]
If you claim rebates for treatment through Private Health Insurance, please be aware that your agreement with your insurance company includes that they may request proof of treatment from us, which is supplied to them as copies of your treatment notes.
